Not only will GSK and Bayer be going up against Pfizer, the world's largest drug company and arguably the industry's best marketer, but they will also be facing another impotence drug due to appear soon: Eli Lilly's Cialis. Gradually Qureshi, who is diabetic, noticed his erections becoming less rigid, and his sex life tapered off. His wife, very much a product of her Muslim upbringing in Pakistan, never uttered a word about the change. "I felt empty, as if I was no longer a man," says Qureshi. He went to doctors, who offered Viagra, but Qureshi says the drug gave him headaches and blurred vision. So he did nothing until winter 2000, when he saw an ad in the Miami Herald seeking men with ED problems for a clinical study. Underwritten by GSK, the study was a double-blind experiment to test Levitra in an "at home" setting. Volunteers were given a four-week supply of either 20-milligram Levitra tablets or a placebo, and sent home. Levitra's European marketing campaign, which began last March in Britain and Continental Europe, focuses heavily on recruiting women. A web promotion for Levitra in the British Isles includes a seven-part primer for women, the first section of which is titled "Honey, it's not you!" It explains that although Hippocrates, the father of medicine, once described the causes of impotence as being "preoccupied with work or having an unattractive wife," modern medicine has found that 85% of ED cases are associated with medical conditions like diabetes, heart problems, or high blood pressure. A section titled "I need your help. Can we talk?" coaches women on how to broach the touchy subject. Levitra's German website sympathizes about fears a woman may feel when faced with a partner's ED. ("Does he still love me?" "Is there another woman?") Drug companies leave nothing to chance when it comes to enticing patients and doctors. The names of drugs, the color and shape of the pills, and the imagery used to sell products are heavily researched and tested, a little like the drugs themselves. In January 2002 the Bayer and GSK marketing groups met face to face for the first time at the Millennium Hotel in New York City. The purpose of the confab was to figure out "how to beat the blues," says Bryan, referring to Viagra's sky-blue tablets. The firm discovered, perhaps conveniently for its client, that consumers didn't "resonate with the imagery" of Viagra, says Bryan. "They find the blue color too cool. They equate it with being sick." Physicians are another key target for Levitra. When a patient comes in for his annual checkup, he's typically poked and prodded and screened for high cholesterol, high blood pressure, and glucose. Baycol was expected to become a multibillion-dollar medicine. But in 2002, after Bayer recruited GSK to help peddle it, Baycol was implicated in 50 deaths and finally yanked from the market. Viagra celebrated a milestone anniversary recently. Just five years ago, in 1998, the little blue pill spawned the market for "erectile dysfunction" drugs with the biggest launch ever in the industry. Pfizer's sales and marketing prowess, coupled with media frenzy and endless Jay Leno punch lines, made Viagra so popular that, a mere year after its introduction, the name was added to the Oxford English Dictionary. Pfizer even got stoic former Senator Bob Dole to plug the product in TV commercials. The marketing onslaught has never let up. In 2000, Pfizer sponsored free erectile-dysfunction (ED) screenings at Nascar events. The next year the company financed a "Cool Blue September" national concert tour, featuring disco-era band Earth, Wind & Fire. Last year Viagra became an official sponsor of Major League Baseball. In all, Pfizer spends an annual $185 million to promote Viagra, according to market researcher Verispan in Newtown, Pa. That includes direct-to-consumer advertising, meetings and events for doctors, advertising in medical journals, and the cost of sales visits to doctors, offices, and hospitals. More than 20 million men (and some women) around the world have tried it; more than a billion of the tablets, which retail for $6 or more a pop, have been sold. In 2002 the "Pfizer riser" had its biggest year ever, garnering $1.7 billion in sales. The big numbers, however, obscure a little-known fact: Viagra is an underachiever. From its notoriety you might think it the drug industry's No. 1 bestseller. It's not; Viagra doesn't even make the top ten. But they vary slightly in their effect. Viagra's biggest failing is that for roughly 30% of men who try the drug, it doesn't work at all. Goldstein may not be entirely impartial, but that's modern medicine: Disinterested experts are hard to come by when many of the leading medical minds also are conducting clinical trials for manufacturers and striving to expand the influence of their field. Goldstein, who has taken part in several major trials for Viagra, Levitra, and Cialis, insists he can be impartial because his earnings from the trials are channeled to Boston University Medical Center, not his bank account. Dr. Myron Murdock, another of Levitra's thought leaders, is a urologist who has helped oversee clinical trials for Viagra and each of its new rivals. "With a lack of food-intake issues and faster onset, Levitra fits into the sexual lifestyle of most men," he says.

Viagra Abuse May Boost Sex Diseases, Officials Warn

San Francisco has petitioned federal regulators to warn that the anti-impotence drugs like Viagra use could increase the risk of sexually transmitted disease and HIV, officials said on Monday. The request to the Food and Drug Administration earlier this month is a response to recreational use of Viagra among gay men who use it to enhance promiscuous sex. "The predominant problem that we see in San Francisco is that widespread use among gay men with multiple partners," said Jeffrey Klausner, director of sexually transmitted disease prevention at the city's health department. "It increases their ability to have more sex partners, particularly in multiple partner environments, and reverses the chemical impotence associated with methamphetamine and crystal use," Klausner said. By petitioning the FDA, San Francisco's Department of Public Health has triggered a 180-comment period for the agency to review the issue. It also comes at a time when Viagra's manufacturer Pfizer is airing a new advertising campaign with a friskier tone to encourage use of the drug. San Francisco Department of Public Health officials seek a health warning on Viagra and similar drugs such as Levitra, sold by GlaxoSmithKline Plc and Bayer AG, and Cialis, sold by Eli Lilly & Company and Icos Corp. The Stop AIDS Project in San Francisco said a third of gay men they interviewed on the street had used Viagra. "It is well-known in the gay community that Viagra is used as a recreational drug," said Darlene Weide, the group's director. In recent years, U.S. public health officials have reported higher rates of sexual disease and HIV infection in the gay community in general. The lure of Viagra, which acts by increasing blood flow, may be especially attractive to some in the gay community because of the nature of homosexual sex. "Some have suggested that anal intercourse requires a firmer erection than either oral sex or vaginal sex," Klausner said. Spokesman Daniel Watts said Pfizer opposed changing the labeling and said the firm had long engaged in a public education campaign, including in a Web site called sexualsmarts.org. Pfizer Inc. on Friday denied sildenafil, sold under the trade name Viagra, may cut fertility in men, shrugging off a study reported this week that found the widely used impotence treatment could hamper sperm's ability to fertilize an egg. "There's no evidence that Viagra decreases fertility in men," Pfizer spokesman Daniel Watts told Reuters. "There have been many clinical trials as well as use by 23 million men in 6 years of use around the world, and there's been no evidence that Viagra affects fertility, so we don't put a lot of stock in that study," Watts added. Researchers from Queen's University in Belfast found that sperm exposed to Viagra became more active. But they said Viagra also speeded up the acrosome reaction, in which sperm release a digestive enzyme that helps to penetrate the outer wall of an egg to allow fertilization. If this process is completed before sperm reaches an egg, it could render them unable to fertilize the egg, researchers said. Sheena Lewis, director of the Reproductive Medicine Research Group at Queens University, called the findings worrying. As Viagra has become a drug used for sexual enhancement as well as for its intended purpose of treating erectile dysfunction, Lewis said, "caution should be taken when using recreational drugs if you are hoping to start a family." The study did not look at Viagra's new rivals--Levitra (vardenafil) from GlaxoSmithKline Plc and Bayer AG or Cialis (tadalafil), sold by Eli Lilly & Co. and Icos Corp.--but both of those drugs work in a similar fashion to Viagra. "It's one study and it was in the test tube basically, not in real people," Pfizer's Watts said. "Viagra has helped some men actually father children, helped aid in the fertility of men who have severe erectile dysfunction," Watts added.

Drug deals cost U.S. consumers $3.5 bln a year: FTC

Reuters - Consumers, insurance companies and the federal government spend an extra $3.5 billion for prescription drugs every year because brand-name companies pay generic producers to stay out of the market, the head of the U.S. Federal Trade Commission said on Tuesday. FTC chief Jon Leibowitz urged Congress to pass legislation, now pending in the U.S. Congress that would ban deals in which brand-name drug makers pay generic companies to delay production of cheaper versions of popular drugs. “Eliminating these deals is one of the Federal Trade Commission's highest priorities,” Leibowitz said at the Center for American Progress, a think tank. While a member of the U.S. Senate, President Barack Obama co-sponsored an earlier version of the legislation. The FTC estimated that the federal government pays about one-third of all prescription drug costs, so eliminating “pay for delay” settlements would save the government $1.2 billion annually. The FTC has challenged the deals in court, saying patent settlements were sometimes used to disguise payoffs that kept generic versions of drugs off the market. The agency has had mixed results in the courts. The first known “pay for delay” was in 1994 when Bristol-Myers Squibb Co paid $290 million to Schein Pharmaceutical to delay the sale of a generic version of Bristol's anxiety drug Buspar. The FTC has calculated that consumers could save even more if generic companies were allowed to make copies of expensive biotechnology drugs, something the U.S. Food and Drug Administration does not now allow. The future will likely offer many new drugs like Viagra: expensive drugs that have lifestyle benefits without actually curing an illness or disease. We might be comfortable drawing a line between paying for curative treatments and those that are "cosmetic," and asking individuals to pay for cosmetic treatments themselves. So is Viagra cosmetic, curative, or both? Paying for lifestyle treatments Fertility treatments like in-vitro fertilization (IVF) are not usually covered by insurance because it is considered medically unnecessary. But it improves life in a way that restores a normal function- offering some infertile women the possibility of having a child.

Neither Viagra nor IVF are like cosmetic surgery, but both can be used in a "cosmetic" way by people who don't have a medical need for them: IVF for women who want to select specific traits for their children, and Viagra for men who think it will add vitality to their sex lives. The difference between using drugs or treatments for medical reasons and cosmetic purposes offers a way to decide which deserve coverage. We should feel even less worried about denying coverage for cosmetic uses when the drug is affordable to most everybody. Protecting access in the future As we live longer and healthier lives, our health care needs will become more about treating chronic effects to our health, such as Alzheimer's disease or impotence, than about treatments for heart attacks in middle age. It is important that we protect our access to drugs and treatments that improve our health by protecting normal functions. The key will be determining what counts as normal, and when the same treatments are merely enhancements. They found that in women, drinking cola was associated with lower bone mineral density at all three hip sites, regardless of age, menopause, total calcium and vitamin D intake, or smoking or drinking alcohol. Women reported drinking an average of five carbonated drinks a week, four of which were cola.

Bone density among women who drank cola daily was almost 4 percent less, compared with women who didn't drink cola, Tucker said. "This is quite significant when you are talking about the density of the skeleton," she said.

Cola intake was not associated with lower bone mineral density in men. The findings were similar for diet cola, but weaker for decaffeinated cola, the researchers reported.

The reason for cola's effect on bone density may have to do with caffeine, Tucker said. "Caffeine is known to be associated with the risk of lower bone mineral density," she said. "But we found the same thing with decaffeinated colas."

Another explanation may have to do with phosphoric acid in cola, which can cause leeching of calcium from bones to help neutralize the acid, Tucker said.

One expert agrees that women should reduce the amount of cola they drink.

"I would expect this finding," said Dr. Mone Zaidi, director of the Mount Sinai Bone Program at Mount Sinai School of Medicine, in New York City. "It's probably a caffeine-related problem."

Women should limit their caffeine intake, Zaidi said. "Caffeine interferes with calcium absorption, which results in less bone formation," he said.

This can be a problem for younger women who never develop peak bone density, Zaidi noted. "Younger women who have a lot of coke will not form bone to an extent their peers would; so, years later, in menopause, they are going to be disadvantaged," he said.

The jury is still out on whether other SSRIs -- such as Celexa, Paxil and Zoloft -- might have the same effect on bone, Battaglino added, since similar tests on those drugs haven't yet been performed.

"This could be a class effect for SSRIs," he said. "However, it is known that in addition to blocking the serotonin transporter, Prozac can target other molecules -- for instance, some nicotinic acetylcholine receptors and even some serotonin receptors. So, this effect could be specific for Prozac. The experiments will have to be done to answer the question."

Competitor Viagra, which operates 24 hours, goes on sale

To bolster the fortunes of his leftist Party of Democratic Revolution and to further his own dream of becoming the country’s president in 2012, Mr. Ebrard has pushed to legalize abortion and gay civil unions in the capital and crack down on illegal street vendors and unlicensed taxi operators, who have long been associated with crowds and crime. His plan to expand subway and bus service is ambitious and popular. In announcing the erectile dysfunction program in November, Mr. Ebrard, 49, portrayed it as a way of bringing smiles to the faces of those who have reached the tercera edad, or third age, as Mexicans call the golden years. “Everyone has the right to be happy,” the mayor said, noting that many of the poorest elderly people do not qualify for employer-based health plans and have been abandoned by their families. “They don’t have medical services, and a society that doesn’t care for its senior citizens has no dignity.” An estimated half of Mexican men over the age of 40 experience difficulties achieving erections, said Dr. Iran Roldan, a specialist in geriatrics who helps run the new program at Mexico City’s Department of Public Health.

But the subject has not been one that many men have felt comfortable talking about before. Getting men into public clinics with the promise of free erectile medicine, Dr. Roldan said, could help them get treatment for other related health problems, like diabetes, hypertension, obesity and depression. “This is a public health problem,” she said. So far, huge crowds have not turned out for the free Viagra, Levitra or Cialis, which are the three tablets being offered. Fewer than 100 inquiries have been made at health clinics and only about a dozen or so men whose erectile dysfunction has been diagnosed have begun the process to get the pills, health officials say. They range in age from their early 60s to 82. Still, the new program has managed to provoke a spirited debate on a topic that was considered taboo before: sex among senior citizens. One of Mr. Ebrard’s long-shot rivals for the presidency, Fidel Herrera, 59, the governor of Veracruz State from the Institutional Revolutionary Party, dismissed the Viagra handouts as ridiculous. “What’s the point of encouraging old people to have sex?” he asked in a recent interview. “There’s such a thing as nature. You can’t play God.” But others disagreed with that take. “Nobody pays attention to us,” said Bernarda Valenzuela, 77, whose husband died in an accident years ago. “Those children who care for their parents only worry about giving them food and changing their clothes, as if we were children. They forget that we feel many things, even sexual things. We’re not made of wood.” But Pepe Castro, 65, a barber who dyes his hair jet black, thought the money spent on the pills could be better used on more pressing matters. “There’s other things more important,” he said. “Everyone wants sex, no matter the age, but the government ought to be paying for medicines to keep people alive, not this.” As for Mr. Posadas, he has used three pills already and has three left from his initial batch. Soon, he will return to the clinic for more tests and, he hopes, another supply. His artificial knee still hurts him and his cholesterol is elevated. But other than that, he said he felt quite robust. “I’ll enjoy whatever time I have left,” he said, flexing his biceps a bit.
